The
RFP must include a cover letter, which must be signed by an individual who=
is
authorized to contractually bind the Bidder. An unsigned cover letter may =
cause
the proposal to be rejected. The letter must also contain the following:

Bidder’s name, address, e-mail, main telephone and facsimile number.=

Bidder’s Federal Employer Identification Number and, if applicable,
Corporate Identification Number.

name and address of any entity that is a parent of or owns a controlling
